1. Have you had any recent changes to your medical history? ---Colonoscopy. Removed polyps. ---Recently diagnosed with IBS
2. On a scale of 1 to 10, how would you rate your current job satisfaction? ---8
3. Have you at any time in the last 3 months experienced periods of abnormal anxiety, anger, sleeplessness, and/or irritability? ---Some sleeplessness, but only due to noisy bunkmates.
4. Have you had any thoughts of harming yourself or others? ---Nothing outside of reprimanding said noisy bunkmates.
5. How would you describe your current work/life balance? ---It could be better. We don't get much vacation time, so I don't get to go home and see my wife and kid as often as I'd like, and that's been hard on us.
6. Do you have any unusual concerns about your coworkers? ---Not really. I would have said something about Hank, but seems that's no longer a problem.
