1. Have you had any recent changes to your medical history? ---Colonoscopy. Removed polyps. ---Recently diagnosed with IBS
2. On a scale of 1 to 10, how would you rate your current job satisfaction? ---9
3. Have you at any time in the last 3 months experienced periods of abnormal anxiety, anger, sleeplessness, and/or irritability? ---Some sleeplessness, but only due to noisy bunkmates.
4. Have you had any thoughts of harming yourself or others? ---Nothing outside of reprimanding said noisy bunkmates.
5. How would you describe your current work/life balance? ---Pretty good! We just got those extra vacation days, so that allows me to spend more time with my wife and kid back home! Thanks to corporate for making that possible!
6. Do you have any unusual concerns about your coworkers? ---Not really. Hank's got a bit of a chip on his shoulder, but the kid seems harmless enough. He'll learn.
